Andrew Wakefield mag zichzelf geen arts meer noemen. De General Medical Council verwijderde hem gisteren uit het Engelse BIG-register.

Antivaccinatie-held Wakefield valt steeds dieper.

Wakefield publiceerde in 1988 in de Lancet een onderzoek waarin hij een verband legde tussen vaccinatie tegen de bof, de mazelen en de rode hond en autisme.

Wakefield werd een held voor de antivaccinatiebeweging. Maar Wakefield zakte steeds dieper. Hoewel er al sinds de publicatie kritiek was op het onderzoek van Wakefield en acht van de tien onderzoekers in 2004 hun handen aftrokken van dit onderzoek, cumuleerde zich het ‘onheil’ voor Wakefield dit jaar.

De GMC beoordeelde zijn onderzoek als oneerlijk en onverantwoordelijk. De Lancet trok het onderzoek in.

Nu is ook zijn artsentitel afgepakt.

Verwacht maar niet beste Cryptolezers dat er iets gaat veranderen in de antivax-beweging. Andrew Wakefield zal misschien nog wel luider bezongen worden. Hij is immers een held die opkomt voor de vaccinatieslachtoffers, de (toekomstig) autistische kindersjes en niet te vergeten hun ouders. Andrew Wakefield zal geëerd worden als de man die in z’n eentje in het geweer kwam tegen de mondsnoerende BigFarma en de gesloten medische stand die dit soort dissidenten veroordeelt.

De wetenschap mag dan nog zo hard roepen dat er geen verband is gevonden tussen autisme en vaccinaties, niets kan de antivaxxer overtuigen.

Dat Wakefield door zijn malicieuze gedrag en dito onderzoek de vaccinatiegraad heeft doen dalen en daardoor vele kinderen heeft opgezadeld met te vermijden ziektes en de complicaties hiervan, mag niet hinderen.

    Wat is het risico van vaccinatie? Wie de tv-show van Oprah Winfrey in 2007 heeft gezien weet het antwoord: autisme, natuurlijk. Dat antwoord is fout. Autisme is een aangeboren hersenafwijking met een sterke erfelijke component. Met vaccinatie heeft autisme niets te maken. Het zijn valse profeten die een verband tussen autisme en vaccinatie blijven verkondigen, zoals Paul Offit in zijn meesterlijke boek Autism’s false prophets (Columbia University Press, 2008) laat zien.

    Een mogelijk verband tussen vaccinatie en autisme zingt al lang rond. Juist tegen de tijd dat kinderen hun eerste prikken krijgen, worden de symptomen van autisme onmiskenbaar, al hebben oplettende ouders vaak al eerder door dat er een probleem is in het contact met het kind. Als ouders de problemen echter pas zien ná de vaccinatie, valt te begrijpen dat ze denken dóór de vaccinatie.
    Die argwaan tegen vaccinatie kreeg een schijnbare wetenschappelijke onderbouwing tijdens een persconferentie in februari 1998. Gastro-enteroloog Andrew Wakefield kondigde daarin aan dat hij een verband had ontdekt tussen autisme en de vaccinatie met het tripelvaccin tegen mazelen, bof en rode hond. Wakefield wist ook hoe het verband in elkaar stak: hij had ontstekingsverschijnselen gevonden in de darm van gevaccineerde autistische kinderen en zijn interpretatie was dat het vaccin een chronische darminfectie had veroorzaakt, waardoor de darm lek raakte. Zo konden toxines het lichaam binnendringen en de normale hersenontwikkeling verstoren.

    De toegestroomde journalisten waren geïmponeerd. Andrew Wakefield was geen halvegare fantast, maar een jonge, charismatische specialist, werkzaam in een academisch ziekenhuis. Bovendien werden zijn resultaten diezelfde dag gepubliceerd in de Lancet, een van de beste medische tijdschriften. De persconferentie resulteerde in grote koppen in de Britse kranten: ‘Verbiedt het tripel-vaccin!!!’ Tegenpruttelende dokters werden weggezet als lakeien van de farma-industrie. Die fabrikanten verdienden 2 miljard per jaar met vaccins en zo’n markt laat je niet lopen vanwege een paar zielige kinderen. De gevolgen bleven niet uit: de vaccinatiegraad in Engeland en Ierland zakte hier en daar tot 50% en kinderen gingen dood aan de mazelen, net als vroeger.

    Toch was in 1998 al duidelijk dat Wakefields waarnemingen weinig bewijskracht hadden. Ook Richard Horton, de Lancet-hoofdredacteur, wist dat. Daarom liet hij twee epidemiologen in hetzelfde Lancet-nummer uitleggen………

Wat zijn bezorgde ouders vaak onnozel, wat komen ze makkelijk tot rotsvaste overtuigingen op basis van minimale aanwijzingen, en wat reageren ze agressief als iemand het waagt om die overtuigingen ter discussie te stellen.     Op 2 februari 2010 kondigde de Lancet aan dat het artikel van Wakefield et al. uit 1998 ingetrokken was. Beter laat dan nooit, maar zoals bovenstaande bespreking duidelijk maakt, er waren voor publicatie al redenen genoeg om het artikel te weigeren. Waarschijnlijk wordt geen antivaccinatiegelovige door deze intrekking overtuigd, maar het doet misschien wel iets af aan de overtuigingskracht van deze beweging.

    Dr. Andrew Wakefield was almost single-handedly responsible for frightening the public about a possible association between autism and the MMR vaccine. His alarmist recommendations directly led to lower vaccination rates and a resurgence of measles to endemic levels in the UK. The MMR/autism interpretation of his 1998 article in The Lancet was retracted by 10 of his 12 co-authors. The article itself was “fully retracted from the public record” by The Lancet. And now Wakefield has lost his license to practice medicine after the General Medical Council’s exhaustive 2½-year review of his ethical conduct.

    If my post today is a bit shorter on the usual Respectfully and not-so-Respectfully Insolent verbiage that you’ve come to know and love (or hate), I hope you’ll forgive me. It’s hard not to sit back, rest a bit, and enjoy the spectacle of Andrew Wakefield being pilloried in the press in the wake of the BMJ’s article documenting his scientific misconduct in gory detail. He’s gotten away far too long with trying to split the difference when credulous journalists “tell both sides of the story” so that to those not knowledgeable about his scientific fraud and incompetence it seems as though there really are two sides to the story of whether or not the MMR vaccine causes autism. I realize that schadenfreude is generally considered a less than honorable emotion, but, you know what? Sometimes it’s not only justified, but necessary.

    This is one of those times.

    Several of you have been sending me this; so I would be remiss not to note that there is a rather lengthy profile of Generation Rescue’s favorite “martyred” anti-vaccine hero, disgraced and discredited British gastroenterologist Andrew Wakefield, in this weekend’s New York Times Magazine entitled The Crash and Burn of an Autism Guru (de koptekst blijkt inmiddels veranderd. red.). After all, there are lots of things that aren’t based on science that are are reasonable, or at least not batshit insane. Nor is it just because anti-vaccine views are clearly based far more on belief than on evidence or science, although that certainly helps.     To a certain extent, I understand the assertion of “once a cheat, always a cheat.” I understand that the lead author (Madsen) and Thorsen’s other co-investigators might now want to check over Thorsen’s contribution to the two papers (as relatively small as it appears to be compared to the other authors), even for the paper whose work was not funded by the CDC at all and therefore has zero financial dependency on the CDC. That’s normal caution. However, normal caution is most definitely not what these attacks by Safeminds and AoA are about. They’re about the denialist technique of spreading FUD (fear, uncertainty, and doubt) about vaccines. Let’s just put it this way. Let’s say the anti-vaccine movement’s wet dream about Thorsen came true and it was somehow discovered that his science was also falsified and that, further, his fraud was enough to call the conclusions of every study for which Thorsen was a co-author in doubt. Even in that highly unlikely scenario, in which both studies were somehow completely discredited as a result of Thorsen’s financial chicanery with grant funds, it would not be nearly enough for scientists to call into question the scientific consensus that neither the MMR nor thimerosal are associated with an increased risk for autism. The reason is that there’s so much other evidence that is consistent with the Danish studies and similarly shows that neither the MMR nor thimerosal in vaccines is associated with autism.

    What AoA, Safeminds, and other denialists refuse to understand is that science is rarely, if ever, a matter of a scientific consensus being based on one study, two studies, or a handful of studies. A scientific consensus is based on examining all the evidence from all relevant studies, deciding which studies are most methodologically powerful, and then synthesizing it all into a conclusion. Contrast this to how the anti-vaccine movement treats its “brave maverick doctors” like Andrew Wakefield, Mark Geier, Rashid Buttar, et al, and the difference between real science and anti-vaccine pseudoscience couldn’t be clearer.

    Here we go again.

    Having been in the blogging biz for nearly seven years and developed a special interest in the anti-vaccine movement, I think I’ve been at this long enough to make some observations with at least a little authority. One thing that I’ve noticed is a very consistent pattern in which, every time a new study or report released that either fails to find evidence that vaccines cause autism or significant harm or that even concludes that vaccines do not cause autism, the anti-vaccine movement is right there, ready to attack it with pseudoscience, misinformation, and exaggerations of the study’s or report’s shortcomings. Indeed, outside of anthropogenic global warming (AGW) denialists, I can’t think of another group of science or medicine denialists besides anti-vaccine loons who have such an efficient “rapid response team.” Inevitably, any time a major study or report is released exonerating vaccines, it’s a sure bet that within 24 hours one of the major anti-vaccine groups will have a press release ready or that wandering home of happy anti-vaccine sycophants, toadies, and lackies, Age of Autism, will have pseudoscience-laden pseudo-rebuttal.

    Whenever I refer to an anti-vaccine activist as an “anti-vaxer” or an “anti-vaccinationist,” I can always count on outraged and self-righteous denunciations from the the person who is being labeled as “anti-vaccine.” “Oh, no,” she’ll say, “I’m not ‘anti-vaccine.’ I’m pro-safe vaccine.” or “I’m a vaccine safety activist.” Of couse, over the years, I’ve learned that the vast majority of such people are deluded in that they probably do really believe that they aren’t anti-vaccine, but everything they do and say is pretty much always anti-vaccine. It’s easy enough to tell just by asking a couple of simple questions. Often only one will suffice, and that question is: “What would it take to reassure you that vaccines are sufficiently safe that you would vaccinate your children?” Quite often, the answers will be levels of safety that are not feasible in the real world, such as……….

    Vaccinaties niet oorzaak autisme

    We hebben het er al verschillende keren over gehad op Kloptdatwel – de angst dat vaccinaties de oorzaak zouden zijn van autisme. Onlangs is er een nieuw onderzoek gepubliceerd waaruit opnieuw blijkt dat er geen relatie is tussen autisme en vaccinaties. Toch zal ook dit onderzoek de anti-vaccinatiebeweging vermoedelijk niet overtuigen. De gevolgen zijn afschuwelijk. Niet ingeënte kinderen zullen onnodig ziek worden en overlijden.

    ‘Mazelen is terug van weggeweest’

    De extreem besmettelijke ziekte mazelen waart al geruime tijd door Europa. Vaccinatieprogramma’s en blijvende inspanningen zijn nodig om deze epidemie te bestrijden. Dat stelt Mark Geels van SciencePalooza.

    Een paar maanden geleden stonden alle autoriteiten en de media op de banken toen onze taugé besmet bleek met de EHEC bacterie. Maar geruime tijd al waart er een andere vergelijkbare dodelijke epidemie door Europa en blijft het doodstil. De score tot nu toe: 14.000 patiënten in Frankrijk, 4330 in Italië, 1480 in Duitsland en een verdriedubbeling in Nederland ten opzichte van 2010. In Europa zijn nu al 400 mensen opgenomen in ziekenhuizen, 24 mensen kregen hersenvliesontsteking en 8 zijn overleden. Het meerendeel daarvan zijn kinderen. Mazelen is terug van weggeweest.

    Whatever Wakefield’s motivation, this move reeks of desperation. After all, if this was such obvious libel, why did Wakefield wait nearly a full year after the publication of Deer’s BMJ articles until filing this lawsuit? In taking this action, Wakefield is both demonstrating the characteristics of a crank (the need to silence using the courts rather than to refute using science) and showing just how far he’s fallen.

    When I wrote last week about the latest legal thuggery against an opponent of antivaccine pseudoscience, this time by hero of the antivaccine movement, who sued investigative reporter Brian Deer for defamation, there was one thing about the case that confused me, one aspect that didn’t add up to me. Part of it was why Wakefield sued Brian Deer over an article he wrote for the BMJ a year ago, although in retrospect it’s become apparent to me that it was almost certainly because the statute of limitations for a libel action in Texas is one year. More importantly, the silence of the antivaccine movement in general and the propaganda organ of the antivaccine group Generation Rescue, Age of Autism, was utterly deafening. Usually, AoA is Wakefield’s biggest cheerleader. As I mentioned before, …….

    In the meantime, we are reminded of the power of vaccination by this story, which points out that it’s been a year since the last case of polio was recorded in India. Although it has been delayed by antivaccine sentiments based on religion and paranoid conspiracy theories, the eradication of polio is now–finally!–potentially achievable. Just as smallpox was eradicated. Compare and contrast: Thousands of cases of measles in the U.K. and Europe, largely thanks to the fear of the MMR stoked largely by Wakefield’s fraudulent research. No matter how much Wakefield’s antivaccine fans try to spin it otherwise, that is how Wakefield will be remembered by history, and, in my opinion, rightly so.
